Try it. Nothing you type here is sent anywhere. On a real booking page, sending this form is a request: the artist reviews it and replies with a quote and dates.
Built from the same parts as a real Maelo booking page. Try it: nothing you type is sent.

About this form
What makes this form different
This is the everyday form for original work. It asks for what you need to decide if a piece is a fit and to put a price on it, and nothing you will never read.
- Placement, size and style come first, because those three decide the quote and the session length.
- Budget and first-tattoo questions are optional. They help you pitch the piece, but nobody should be stopped by them.
- It ends with an age check, so you never quote someone you cannot tattoo.
One question, rewritten
Small wording, very different answers
"Size?" gets you "medium" or "palm sized". Two numbers and permission to guess gets you something you can quote.
Size?
Rough size, length and width. A best guess is fine.
Setting it up in Maelo
From template to booking page
In Maelo the name, email, title and description are already on every form. You add the rest by dragging in a Text Field, Size, Dropdown, Image Upload, Date Picker and Checkbox, then reword each one.
A sent form arrives in your requests inbox with every answer and photo attached. It is a request, not a booking: you review it, then reply with a quote and dates. The booking form guide covers every field and its settings.
