About this form
What makes this form different
A cover-up request is about two tattoos: the one they have and the one they want. This form gets the facts about the first before you start designing the second.
- Photos of the existing tattoo are required. You cannot judge a cover from a description.
- It asks whether they want it covered or reworked, because those are different jobs.
- It asks how dark they are willing to go before you design anything they will turn down.
