Ask what you
actually need.
Build a tattoo booking form around the work you want to take. Ask for the idea, placement, size and reference photos, with your own wording and required questions. Every answer arrives together in your requests inbox.

That is the form your client fills in. Not a generic template.
Field types
A question type for every kind of answer
Some answers you want in the client's own words. Some you want to be able to count. A free text box gets you a paragraph you have to read; a dropdown gets you an answer you can compare across requests.
So there is a block for each: a short answer and a long one, a number, a rough size in inches or centimetres, a dropdown, a multi choice, a tickbox, reference photos, files, which of your locations they want and which days suit them.
Every question carries its own wording, its own hint line, whether it is required, and how wide it sits. Nothing on the form below is a picture. It is the booking form's own components, filled with the answers a client would give.
The booking form guide covers each field and its settings. Once a client submits, you review the request and offer a quote before they choose from the dates you offer.
